MAG Model Appraisal Form
The MAG Model Appraisal Form is intended as an example of a repository that holds the information required for a medical appraisal. It has been designed with the appraisal meeting in mind, in a logical manner that mirrors how the appraisal conversation may flow.
The form is intended as a practical demonstration of the information in the Medical Appraisal Guide. It is an interactive pdf, which allows you to type information into boxes and upload documents into the form.
Information for doctors
Doctors should complete up to and including Section 17 and submit the package of information to the appraiser by a mutually agreed date. Sections 18, 19 and 20 will be completed during and immediately after the appraisal meeting by both the doctor and the appraiser.
The MAG Model Appraisal Form User Guide has also been developed to help doctors navigate their way through the form.
Information for designated bodies
Designated bodies using the MAG form will need to design their appraisal systems locally. This should include describing arrangements for safe and secure storage of the form and its contents, verification of the connection between the doctor and the content of the form, and safe and secure transfer of the form and its contents between doctor, appraiser and responsible officer.
The RST does not provide technical support for the form, so designated bodies will need to provide both user guidance and IT support to their doctors and appraisers.
Advice on the support the form requires, and the IT platforms necessary to run it are provided in the Organisational Guidance for using the MAG Model Appraisal Form.
Designated bodies should also consult Information Management for Revalidation in England.
